Age Cut Off

Beginning January 1, 2023, our practice will be implementing an age cutoff. We will no longer be providing care to patients over the age of 19.

This decision was made due to our current patient volume. We have sent multiple letters to inform patients who are affected by this change. If you have not received one of these letters and were a current patient, please contact our office for more information and assistance with transferring your records to a new primary care provider.
